Travel Consent for Minor

A parent or guardian's signed authorization for a minor child to travel. Commonly required for school trips, international travel, and travel with non-parent adults.

If you're NOT traveling — what does the escort need to bring?

The notary verifies your identity when you sign, but you don't need to be at the airport. The escort (the adult traveling with your child) carries the documents. The escort should travel with all of the following:

  • The original notarized travel consent (this document, with notary seal)
  • A certified copy of the minor's birth certificate showing your name as parent — certified copy means an official copy from the vital records office with raised seal/stamp, NOT a photocopy
  • The escort's government-issued photo ID
  • The minor's passport (required for international travel)
  • A copy of YOUR photo ID (parent's ID — helpful for verification)

For international travel: destination country may require the OTHER parent's signed consent too, plus an apostille and/or translation. Check your destination's specific requirements before you travel.
