Third-Party Residency Affidavit

Signed by the landlord, host, or property owner — not by the resident. Used when a third party (your landlord, a family member who owns the home, etc.) needs to swear that you live at their address.

Important — read before buying: Each state's DMV may require its own specific form (Florida DHSMV does). Check with the requesting agency before using this document for a driver license or state ID application. This affidavit works for many other purposes (school enrollment, employment verification, court filings, general residency proof) but it is not marketed as DMV-accepted.

Affiant — Landlord, Host, or Property Owner

This is the person who will sign the affidavit in front of the notary. Their signature is what gets notarized.

Residence address

The address where the applicant lives (this is also the affiant's address, since the affiant provides the housing).

Applicant — person whose residency is being attested

The applicant does not sign this document. The landlord/host signs and swears that the applicant lives at the address.
