Before signing

Who signs and what to bring

The landlord / host (the affiant) is the one who signs in front of the notary. They are swearing, under oath, that the applicant lives at their address. The applicant does not sign.

The landlord/host brings government-issued photo ID

Driver's license, passport, or state ID — required for the notarization. The ID is checked against the landlord, not the applicant.

The exact address

Street, apartment number, city, state, ZIP. Spelled the way you want it on the document.

Applicant's name and date of birth

Spelled exactly as it appears on the applicant's other documents.

When the applicant moved in

A specific date is best; an approximate month and year is acceptable for many recipients.

State DMVs and other agencies often have their own specific residency-proof forms. Check with the requesting agency before relying on this affidavit alone.
